The Utah Arts Council is requiring all individuals applying for grants to have a DUNS number. We are currently tracking artistic communities and artists in Utah. By requiring a DUNS number, we can more accurately pinpoint and support artistic endeavors throughout the state.

PLEASE FOLLOW THESE INSTRUCTIONS
WHILE YOU FILL OUT YOUR ONLINE APPLICATION:

1. Go to Dun and Bradstreets Number for Organizations
2. Select “click here to request your DUNS Number via the web”
3. Select your country (United States)
4. Fill in your company information
5. Choose “submit”

a. This will execute a search to see if your organization is already with Dun and Bradstreet

6. If your organization does not show up on the next page, choose “Request a new D-U-N-S Number”

7. Fill in the information required:

a. Primary SIC Code (how to find it)

i.	Click on “?”
ii.	Find appropriate category for your organization
iii.	Once you have the SIC number and description, close SIC window. This should bring you back to the DUNS application page.
iv.	Enter SIC Code
v.	Enter SIC operations description

b. Number of Employees: This should be at least 1
c. Fill out rest of page

8. Choose “submit your request”
9. Verify information on next page
10. Choose “U.S. State Contractor or Grantee” (this is the 4th option shown on the web page)
11. Read next statement
12. Click “yes”
13. Read 2nd statement
14. Click "yes" THIS IS IMPORTANT!
14. Chose “submit your request”

* Your DUNS number will be provided via email within 30 days.
* Once you received your DUNS number, please call the Utah Arts Council and ask that this number be added to your application.
