The 2008 State Legislature enacted a bill that requires Utah Arts & Museums to establish a program creating a Certified Utah Museum designation. Each Utah museum is required to complete the form below indicating that it complies with the basic requirements. Museums must be certified to be eligible for OMS grants.
There are two categories:
1. A museum that is governed by a parent organization (government entity or university) is required to provide an Employer Identification Number (EIN) and identify the parent organization. DUP museums that use the International Daughters of Utah Pioneers EIN are not required to submit this number.
2. A museum that has tax-exempt status (501) (c) (3) with the Internal Revenue Service must include a copy of the letter from the Department of the Treasury confirming that the museum is incorporated as a nonprofit organization and has been assigned an Employer Identification Number.
Upon receipt of the completed Certified Utah Museum form and additional required materials, the Utah Office of Museum Services will provide the museum with a letter designating it a Certified Utah Museum. For those museums that register with the Commerce Department for soliciting funds, the Certified Utah Museum designation reduces the annual registration fee from $100 to $65.

Our staff works with the museum after the self study and provides recommendations that address problem areas or areas of critical need. After a museum has complied with the recommendations and has met the goals, we will provide "Performance Goals Certification" to the museum.
